Job Summary
Case Manager responsible for coordinating all aspects of a young person’s care within residential care programs, including leading case planning, stakeholder engagement and therapeutic practice; collaborate with residential teams to promote placement stability, wellbeing and positive life outcomes; develop, implement and review individual behaviour support and case plans; advocate for the needs and best interests of children and young people; provide practice guidance and support to residential care staff; participate in rotating on-call after-hours support; ensure thorough documentation and reporting; travel across South-East and South-West regions to support multiple locations; require strong communication, organisational skills and the ability to work with children and young people who have experienced trauma and adversity.
Required Qualifications
- Experience undertaking case management within residential care, child protection, youth services or a related field.
- Strong understanding of trauma-informed and therapeutic care principles.
- Knowledge of the Child Protection Act, Standards of Care, and Out-of-Home Care requirements.
- Relevant qualifications in Human Services, Social Work, Psychology, Community Services, Behavioural Science or related discipline.
