Job Summary

Coordinate crisis assessment, triage scheduling, and daily appointment management for the Assertive Community Team serving clients with significant mental health symptoms and functional impairments. Provide phone support, manage resource acquisition, and facilitate referrals while maintaining client charts, writing progress notes, and completing quality assurance activities. Deliver interventions per Individual Crisis/Support Plans, ensure HIPAA compliance, and conduct critical incident analysis. Work afternoon and evening shifts with 24/7 on-call rotation for emergency response.

Required Qualifications

  • High school diploma or GED
  • 21 years of age or older
  • License required based on education and standards determined by educational/degree guidelines
  • Ability to problem-solve in a creative and independent fashion
  • Ability to intervene in a crisis in a calm, controlled, and effective manner
  • Ability to assess and resolve problems for on-call responsibilities
  • Flexibility in work hours
  • Availability for coverage (24 hours a day/7 days a week)
  • Ability to be recalled to work on short notice
  • Ability to remain on duty until the need is resolved
  • Ability to respond professionally to a high frequency of calls from SpringPath clients & interfacing agencies
  • Ability to communicate, receive, and write information clearly and accurately
  • Ability to complete general office duties
  • Ability to treat clients in a respectful manner and conduct services with regard to client dignity and self-determination
  • Ability to adhere to Data Privacy Practices/HIPAA
  • Ability to write progress notes
  • Ability to monitor and maintain client charts
  • Ability to provide objective verbal and written critical incident analysis
  • Ability to document essential activities related to welfare and safety
  • Ability to maintain service standards
  • Ability to achieve client specific and program outcomes
  • Ability to participate in the performance evaluation process
  • Ability to increase knowledge and skills based on client and program needs
  • Ability to teach others
  • Ability to seek supervisory oversight
