Job Summary

Provide strength-based case management to individuals with chronic homelessness and complex psychosocial needs through face-to-face, telephonic, and community-based engagement. Build therapeutic relationships, conduct comprehensive assessments, and support clients in developing independent living skills such as budgeting and lease understanding. Develop and maintain individualized service plans and housing success plans using evidence-based strategies like Housing First and Motivational Interviewing. Coordinate with multidisciplinary teams to connect clients to community resources, assist with benefit applications, and support care transitions. Recognize behavioral health presentations, apply risk assessment practices, and document all activities in the electronic medical record system. Participate in team meetings, housing inspections, and monthly food pantry operations while maintaining required certifications.

Required Qualifications

  • High school diploma or equivalent
  • Valid Ohio driver's license
  • Access to an insured vehicle
  • Nonviolent Crisis Intervention certification
  • At least three years of case management or care coordination experience

Desired Qualifications

  • Bachelor's degree in Social Work, Psychology, or a mental health-related field
  • Two years of care coordination or case management experience (if degree is in a related field)
  • Active Community Health Worker certification
  • Prior experience working with adults with serious and persistent mental illness
