Job Summary
Provides intermittent home-based speech language pathology services under physician orders and plan of care, with supervision by the Director of Clinical Services/Nursing Supervisor. Responsibilities include conducting evaluations, writing treatment reports, implementing the full range of Speech Language Pathology Services as ordered, supervising assigned personnel, educating patients and families on home programs, and coordinating with physicians. Requires licensed SLP with master’s degree (or equivalent practicum/experience), state licensure, and typically 2 years of experience preferred. Travel to patient homes is required; work environment includes indoor office and patient homes in AL. Supervisory and documentation duties (OASIS, progress notes, discharge summaries) are included, along with participation in inservice programs and QAPI activities.
Required Qualifications
- 1. A person who has a master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or
- 2. A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national approved examination.
- 3. Currently licensed in the state(s) in which practicing.
