Job Summary
Provide speech-language pathology services to patients in the home or facility. Evaluate, direct and provide speech/language pathology services; develop and periodically review the Plan of Treatment and Plan of Care; assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests; observe, record, and report changes; provide instruction on alternative communication systems when appropriate; counsel patients, families and healthcare staff; maintain documentation and submit reports; participate in care coordination and discharge planning. Requires minimum six months of experience as a speech therapist / speech language pathologist, and a valid driver’s license, auto insurance, and reliable transportation. Home Health experience is required.
