PRN Speech Language Pathologist SLP

As a Home Health Speech Language Pathologist, evaluate, direct and provide speech/language pathology services to patients in the home or facility. Develop and periodically review the Plan of Treatment and Plan of Care; assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests and applications of therapeutic treatments including audiology screening. Observe, record and report changes in patient condition to supervisor/physician; provide instruction and training to patients in alternative communication systems when appropriate; counsel patients, families and healthcare staff; maintain and submit documentation and reports; participate in care coordination and discharge planning. Requires minimum six months of experience as a speech therapist/speech language pathologist, and a valid driver\'s license with auto insurance and reliable transportation. Home Health experience preferred. Location: Rocky Hill, CT.
