Job Summary
Conduct screenings, evaluations, re-evaluations, discharge planning, and revise established plans of care under physician supervision. Maintain an active state license and hold certification as required by state and national board associations. Collaborate with the therapy team to help seniors achieve independence and quality of life within a modern campus featuring independent living, assisted living, memory care, and skilled nursing services.
Required Qualifications
- Master's degree in Speech Language Pathology from an accredited program
- Certification and licensure requirements as established by state and national board associations
- Active state license
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines
- Completion of all required screenings, certifications, and licensure
