Job Summary
PRN Social Worker provides psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities; conducts psychosocial assessments, leads care plan meetings with interdisciplinary team, maintains EMR documentation, and supports discharge planning and community resource connections. Licensed Master’s level social worker with ability to coordinate with CMS guidelines, QAPI, and dialysis-related education; requires travel between facilities and strong communication skills.
Required Qualifications
- Master’s degree in social work
- Must hold a valid state license; clinical not required
- Unencumbered driver’s license
