Job Summary
PRN Registered Nurse (RN) for home care with Trach care experience. Responsibilities include meeting new/prospective clients for initial assessment (virtual and/or in-person), developing and updating a Plan of Care, communicating with care managers, caregivers, office staff and other medical professionals, consulting with Clinical Director for supervision of services, training clients regarding their care, maintaining clinical records in compliance with state and federal laws, conducting client and caregiver evaluations at specified intervals, providing staff training and clinical oversight. Requires MD RN license with Trach care experience, CPR and First Aid certification, TB and Hep B vaccination, nursing program graduation, clean background and driving record, and ability to travel to cases. Prior home care experience preferred; strong interpersonal skills and comfort with EMR systems are advantageous.
Required Qualifications
- RN State License – MD with Trach care experience
- Current CPR and First Aid certification
- TB vaccination and Hep B vaccination
- Graduation from an accredited and approved nursing program
- Clear criminal background and driving record
- Two professional references within the healthcare field
- Prior home care experience is strongly preferred
- Self-starter with the ability to work independently and manage time effectively
- Strong interpersonal skills, including a willingness to teach and mentor caregivers
- Comfort with technology; prior experience with electronic medical records preferred
- Ability to drive and travel to cases as assigned
