Job Summary
TheraCare Home Health RN—PRN in Central Austin/Round Rock, TX. Manage a patient caseload in the home health setting, perform Start of Care, Recertification, and Resumption of Care assessments, develop and update plans of care, coordinate with physicians and interdisciplinary team members, supervise LVNs and Home Health Aides, participate in IDG/IDT meetings, monitor progress, educate patients and caregivers on disease management and home safety, and maintain accurate clinical documentation to ensure regulatory compliance. Flexible, field-based responsibilities with emphasis on independent decision-making and quality outcomes.
Required Qualifications
- Current Registered Nurse (RN) license in the state of Texas
- Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing
- Minimum 2 years of RN clinical experience; home health experience preferred
- Strong knowledge of home health regulations, OASIS documentation, and care planning
- Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred
- Ability to manage a patient caseload and work independently in a field-based environment
- Experience coordinating care with interdisciplinary teams (PT, OT, ST, MSW, HHA)
- Strong clinical assessment, documentation, and critical thinking skills
- Excellent communication and interpersonal skills with patients, families, and physicians
- Reliable transportation with valid driver’s license and auto insurance
- Proficiency with EMR systems (HomeCare HomeBase preferred)
