Job Summary
The RN Case Manager at Empath Health oversees patient care by creating and maintaining care plans and serving as the primary contact for patients and families during their hospice experience. Responsibilities include reviewing and updating physician orders and educating patients and families about the disease process and palliative care options. Candidates must hold a valid Florida RN license, possess CPR certification, and have at least one year of active nursing experience, preferably with terminally ill patients.
Required Qualifications
- Valid Florida license as a Registered Nurse
- Valid Florida State Driver’s License
- Valid CPR certification from the American Red Cross or American Heart Association
- Minimum of 1 year of current active nursing experience
Desired Qualifications
- Experience in caring for terminally ill patients or those with life threatening illnesses
- Experience in an acute or post acute care setting
- Ability to type 25 wpm
Additional Requirements
- Must have reliable transportation
- Must provide proof of valid automobile insurance
- Required to work in other Care Centers based on business needs
