Job Summary
Coordinate Start of Care, re-certification, discharge, and resumption of care for home care patients. Provide direct patient care by administering medications, performing diagnostic tests, and monitoring vital signs. Collaborate with physicians to develop and implement care plans while educating families on conditions and preventive measures. Assist with discharge planning and offer specialized care for patients with dementia or cognitive impairments. Requires valid RN license, minimum two years of home care experience, and a clean background check. Pay is $50.00 per hour plus mileage.
Required Qualifications
