PRN RN Case Manager

Job Summary
Develop and evaluate individual patient care plans within the home setting while serving as the liaison between the agency and patient families. Manage day-to-day clinical activities, coordinate care authorizations, and oversee implementation of physician orders through regular home visits and team consultations. Ensure accurate documentation, submit weekly billing information, and maintain client data in QuickBase. Provide education on benefits and coordinate with DME and ancillary services to support treatment goals. Build rapport with families, coach field staff on care standards, and attend monthly case management meetings. Maintain open communication with nursing leadership and other departments regarding client status or staffing issues.
Required Qualifications
- Current RN licensure
- Minimum 3 years progressively responsible clinical experience
- Must possess a professional, courteous, compassionate and customer-oriented demeanor
- Must be discrete, trustworthy, and possess a high level of integrity
- Must be able to do bending and lifting required for effective patient related care
- Must be able to safely lift 40-50 lbs
- Must dress in a professional manner when visiting clients (scrubs and nametag preferred)

- Prior management or leadership experience
- Ability to work independently, multi-task, prioritize, and function in a team environment
- Excellent computer and internet skills
