PRN Renal Dietitian - SIGN ON BONUS

Job Summary
Conduct initial and ongoing nutrition assessments, develop individualized care plans, and adjust interventions based on lab results and dialysis adequacy. Provide tailored nutrition education and counseling to End Stage Renal Disease patients and caregivers while collaborating with the interdisciplinary team. Monitor patient progress, document monthly updates, and participate in care conferences and quality improvement meetings. Travel to assigned facilities to provide coverage as needed and maintain credentials in compliance with CMS regulations. Requires a Registered Dietitian with at least 12 months' clinical experience, state licensure, and Bachelor's degree. Minimum of two monthly shift pick-ups available.
Required Qualifications
- Bachelor's degree in Nutrition, Dietetics, or similar area
- Registered Dietitian with at least 12 months' clinical experience
- State licensure or certification
- Intermediate computer skills
- Proficiency with MS Word
- Proficiency with Excel
- Proficiency with Outlook
- Proficiency with Concerto specific applications within 60 days of employment
