Job Summary
Conduct initial and ongoing nutrition assessments, develop individualized care plans, and update them in collaboration with the interdisciplinary team. Design and adjust nutrition interventions based on lab results and dialysis adequacy, then monitor progress and document monthly. Provide tailored nutrition education to End Stage Renal Disease patients and authorized caregivers, considering each patient's medical, social, and economic background. Participate in care conferences and quality improvement meetings while traveling to assigned facilities for coverage. Maintain credentials, stay current with clinical practices, and adhere to CMS regulations and company standards. This role supports Concerto's mission of reimagining dialysis care through a centralized, continuous model for those with end-stage renal disease.
Required Qualifications
- Bachelor's degree in Nutrition, Dietetics, or similar area
- Registered Dietitian with at least 12 months' clinical experience
- State licensure or certification
- Intermediate computer skills
- Proficiency with MS Word
- Proficiency with MS Excel
- Proficiency with MS Outlook
- Proficiency with Concerto specific applications within 60 days of employment
