PRN Renal Dietitian

Conduct initial and ongoing nutrition assessments, develop individualized care plans, and update them in collaboration with the interdisciplinary team. Design and adjust nutrition interventions based on lab results and dialysis adequacy, while monitoring progress and documenting monthly. Provide tailored nutrition education to End Stage Renal Disease patients and authorized caregivers, considering each patient's medical, social, and economic background. Participate in care conferences and quality improvement meetings, traveling to assigned facilities to provide coverage as needed. Maintain credentials and adhere to CMS regulations and company standards. This role requires a Registered Dietitian with at least 12 months' clinical experience and state licensure.
