Job Summary
Develop and initiate individualized plans of care in partnership with patients, caregivers, and physicians, while providing preventative and rehabilitative nursing procedures as ordered. Deliver patient and caregiver education, counseling, and discharge planning, preparing clinical notes and updating the primary physician regularly. Coordinate home health services with community partners and participate in quality assessment, performance improvement, and in-service training. Supervise home health aides, LPNs, LVNs, and CNAs every two weeks, ensuring necessary equipment and supplies are available. Perform on-call duties and maintain a cheerful disposition while driving to patient locations. Requires current nursing license, two years of experience with one in home care, CPR certification, and a valid driver's license.
Required Qualifications
- Must have graduated from an approved school of nursing
- Must be licensed in the state where practicing
- Minimum of two years of experience
- Must maintain a current CPR certification
- Must have and maintain a valid driver's license
- Must maintain automobile insurance coverage
- Must have access to an automobile
- Self-directed and able to work with minimal supervision
- Demonstrates excellent observation
- Demonstrates excellent problem solving
- Demonstrates excellent verbal and written communication skills
- Demonstrates nursing skills per competency checklist
- Shows ability to organize and prioritize workload independently
- Able to lift, position, or transfer patients
- Able to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- Must comply with all training requirements
Desired Qualifications
