Job Summary
Registered Nurse (RN) in Michigan for Home Care at Munson Healthcare. Manages an assigned caseload from admission through discharge using a framework of assessment, planning, coordination, and evaluation; performs comprehensive assessments (including OASIS when required); develops and updates individualized plans of care; provides skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs. Educates and counsels patients and families on disease processes, symptom management, and self-care; coordinates care with an interdisciplinary team; supervises LPNs and paraprofessionals; communicates changes to physicians and team members; secures equipment and supplies; maintains thorough documentation in the electronic medical record. Vaccination/immunity is required; valid Michigan RN license; driving ability with vehicle; background checks and travel to patient homes in various weather conditions.
Required Qualifications
- Licensed RN in the State of Michigan
- Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions
- Able to meet Home Health and Medicare standards through criminal background check and fingerprinting
- Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella; flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption
