PRN Registered Nurse - Home Care

PRN Registered Nurse – Home Care role responsible for managing an assigned caseload from admission through discharge using assessment, planning, coordination, and evaluation; performs comprehensive assessments (including OASIS when required), develops and updates individualized plans of care, and provides skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs. Educates patients and families on disease processes, symptom management, and self-care; coordinates care with an interdisciplinary team; supervises LPNs and paraprofessionals; communicates changes in patient condition to physicians and team; ensures equipment and supplies are secured and maintains thorough documentation in the electronic medical record.
