Job Summary
Conduct home health physical therapy assessments to evaluate patient functional status, mobility, endurance, and pain. Develop and implement treatment plans aimed at improving patient well-being, then treat patients to relieve pain and restore function. Instruct patients and caregivers on care plans and progress while observing and reporting condition changes to the supervising nurse or physician. Complete progress notes for every visit and submit evaluations per LHC policy. Requires current Physical Therapy licensure, CPR certification, and a dependable vehicle. Annual target pay ranges from $82,048 to $123,072 with per-visit point compensation.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Candidates are required to pass a drug test before beginning employment
