Job Summary
Conduct home health physical therapy assessments evaluating patient functional status, mobility, endurance, and pain to develop treatment plans. Perform interventions to relieve pain and restore function while instructing patients and families on care goals and progress. Report patient responses and condition changes to supervising nurses or physicians, completing progress notes for each visit per policy. Treat patients in accordance with state laws and the plan of care, documenting outcomes and projected care frequency. Requires current Physical Therapy licensure, CPR certification, and reliable transportation. Annual target pay ranges from $82,048 to $123,072 with per-visit point pay.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Pass a drug test before beginning employment
