Job Summary
Conduct patient assessments for functional status, mobility, and pain to develop therapy plans of care. Deliver interventions to improve well-being, relieve pain, and restore function while observing and reporting patient responses to the supervising nurse or physician. Instruct patients and caregivers on care plans, limitations, and progress goals. Visit patients per treatment plan, complete progress notes for each visit, and submit evaluations adhering to state guidelines and LHC policy. Report outcomes, goals, and projected care frequency. Requires current Physical Therapy licensure, CPR certification, and a dependable vehicle.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Candidates are required to pass a drug test before beginning employment
