Job Summary
Conduct patient assessments for functional status, mobility, endurance, and pain to develop therapy plans of care. Perform interventions to improve patient well-being, relieve pain, and restore function while observing and reporting treatment responses to supervising nurses or physicians. Instruct patients and caregivers on care plans, functional limitations, and progress goals. Visit patients according to treatment plans, complete progress notes per LHC policy, and report evaluation outcomes and projected care frequency. Requires current Physical Therapy licensure, CPR certification, and a dependable vehicle. Annual target pay ranges from $82,048 to $123,072 with per-visit point compensation.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Candidates are required to pass a drug test before beginning employment
