Job Summary
Conduct home health physical therapy assessments to evaluate functional status, mobility, and pain, then develop and implement treatment plans to restore patient function. Treat patients to relieve pain and maintain maximum functioning while instructing families on care goals and progress. Report patient responses and condition changes to supervising nurses and physicians, completing progress notes for every visit per LHC policy. Submit evaluation outcomes and projected care frequency within state-specific guidelines. Requires current Physical Therapy licensure, CPR certification, and a dependable vehicle. Hourly pay ranges from $39.45 to $59.17 with per-visit point pay between $47.34 and $71.00. Part-time and PRN positions available with comprehensive benefits including 401k and equity stock purchase.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Candidates are required to pass a drug test before beginning employment
