Job Summary
Administer physical therapy to patients in their residences based on physician orders and plans of care. Perform skilled procedures, conduct initial evaluations, and instruct patients and families on in-home activities. Participate in care planning, quality assessment, and performance improvement activities while maintaining clinical and progress notes. Submit OASIS assessments and consult with physicians regarding treatment changes. Direct personnel as required and attend scheduled patient care conferences.
Required Qualifications
- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
- Currently licensed in the state(s) in which practicing.
Desired Qualifications
- Two (2) years' experience, preferred.
