Job Summary
Provide rehabilitation services as a PRN Physical Therapist in a skilled nursing facility, conducting comprehensive evaluations, developing and implementing individualized treatment plans, and delivering one-on-one therapy to residents to maximize mobility, safety, and independence. Collaborate with physicians, nurses, occupational therapists, and speech-language pathologists to coordinate care, educate residents and families on exercise programs and safety techniques, document services per facility and regulatory requirements, participate in care conferences and team meetings, and contribute to a positive, professional care environment. Emphasizes evidence-based practice, frequent assessment of progress, and adherence to infection control and safety protocols.
Required Qualifications
- Current state licensure as a Physical Therapist
- 1+ years of clinical experience in a skilled nursing facility or similar healthcare setting
- Strong clinical reasoning and decision-making skills
- Ability to build rapport with residents
- Excellent interpersonal and communication skills
- Commitment to resident-centered care and empathy
