Job Summary
PRN Physical Therapist (PT) at CommuniCare Rehab – In-house team-based therapy role supporting Hillcrest Healthcare Center residents. Evaluate, review, and screen residents' medical charts and physician-ordered evaluations; develop an individualized treatment plan and implement it through direct treatment and supervision/education of PTAs. Lead interdisciplinary meetings to relay clinical findings and make rehab recommendations, and provide staff/family training when appropriate. Opportunity to serve across multiple centers, enabling broad patient-reach and continuity of care; dedicated to improving quality of life for residents in post-acute care settings.
Required Qualifications
- Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program
- Hold a valid or pending State Physical Therapy License
