PRN Physical Therapist (PT) opportunity at CommuniCare Rehab with Ivy Woods Healthcare Center. Responsibilities include evaluating, reviewing, and screening residents' medical charts; developing and implementing individualized treatment plans; providing supervision for PTAs; leading interdisciplinary meetings; and educating staff and families. Required Qualifications
