Job Summary
Evaluate patients, establish functional treatment plans, and progress therapy to restore function and minimize disability. Conduct initial evaluations, interpret prescriptions, and provide periodic written communication to physicians. Manage discharge planning, patient follow-up, and supervision of rehabilitation aides and physical therapist assistants. Document all treatments, progress notes, and discharge summaries in compliance with state practice acts. Build professional relationships with referral sources including physicians, PAs, and case managers. Maintain state licensure and complete all compliance standards. Accompany patients to physician visits and participate in community outreach events such as health fairs and screenings.
Required Qualifications
- Degree from an accredited Physical Therapy Program
- Current Professional licensure as a Physical Therapist
- Current CPR Certification
- Ability to read, write/type and speak English proficiently
- Ability to see at close range, distance vision, peripheral vision, depth perception, and the ability to adjust focus
- Ability to speak and understand
- Ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
- Ability to stand, walk, sit, use hands to feel, and reach with hand and arms
- Ability to lift and/or move up to 20-25 pounds
- Fine hand manipulation (keyboarding)
- Maintain state license
