Job Summary
Administer physical therapy to patients in their residences per physician orders and the Director of Clinical Services. Conduct initial evaluations, histories, and skilled procedures while improving or minimizing residual disabilities. Develop and revise patient care plans, instruct families on in-home activities, and consult with physicians and agency personnel. Submit OASIS assessments, progress notes, and ICD-10-coded reports within defined time frames. Participate in care conferences, quality assessment activities, and in-service training. Requires current state licensure, two years experience preferred, and ability to lift heavy objects while standing for extended periods.
Required Qualifications
- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association
- Currently licensed in the state(s) in which practicing
- Ability to exercise initiative and independent judgment
- Acceptance of philosophy and goals of this Agency
- Ability to participate in physical activity
- Ability to work for an extended period of time while standing and being involved in physical activity
- Heavy lifting
- Ability to do extensive bending, lifting, and standing on a regular basis
