Job Summary
Evaluate residents and develop individualized physical therapy plans of care focused on mobility, strength, balance, safety, and functional independence. Provide skilled PT services, collaborate with PTAs, nursing, and physicians, and offer resident and family education as appropriate. Complete timely documentation using Net Health and PointClickCare while supporting safe discharge planning and continuity of care within an in-house rehab environment. This flexible PRN role serves a skilled nursing and long-term care population at Aventura at Shiloh Springs in Trotwood, OH.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Ohio Physical Therapist license in good standing
Desired Qualifications
- SNF/LTC experience preferred
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong communication and teamwork skills
- Dependable, compassionate, and resident-focused
