PRN - Physical Therapist Assistant

The Physical Therapist Assistant will implement the plan of care as established by the RPT and physician, carrying out treatment programs in collaboration with the Registered Physical Therapist. Responsibilities include screening residents quarterly for declines in function, modifying treatment plans in consultation with the RPT, monitoring and reporting progress, assisting in re-assessment and discharge planning, supervising Rehab Therapy Aides, educating staff, family and caregivers, identifying and implementing program development needs, coordinating equipment and documentation, coordinating in-services for facility staff, attending required meetings, promoting the profession within the facility, and pursuing continuing education specific to the position. Maintain professional behavior and adhere to company policies and procedures; participate in staff education, in-services, and special events such as National Physical Therapy Month. Knowledge of Functional Pathways policies, clinical programs, and application skills across therapy disciplines is required; CE courses and ongoing professional development are encouraged.
