Job Summary
Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions, determine physical, mental, and social needs, and take patient vital signs. Perform clinical duties including taking history, examining, and determining diagnoses while observing OSHA Universal Precautions. Provide written documentation of patient visits per NCQA standards, place case management referrals, and communicate with patients, caregivers, agency nurses, and other providers to assure proper diagnosis. Maintain patient confidentiality, attend required meetings, and participate in professional development activities. Requires an active nurse practitioner license, BLS certification, and ability to drive up to 150 miles daily in Sussex County, NJ.
Required Qualifications
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
- Active BLS Certification
- Current enrollment in Medicare/Medicaid
- Must maintain a valid driver's license and good driving record
- Outstanding EHR skills
- Location Requirement:To be considered for this position, candidates must reside within Sussex County, NJ
- Full range of body motion including handling/lifting patients. Manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking and stair climbing
- Regular lifting/carrying items weighing up to 50 pounds
- Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings. Ability to drive, if necessary
