Job Summary
Provides support and assessment of client needs, and facilitates access to treatment, rehabilitation and service linkage, with case monitoring and client advocacy. Responsibilities include transportation coordination, facilitating group meetings and taking meeting minutes; supervising and scheduling support associates; developing wellness recovery action plans and plans of care with client/family consensus; administering medications; monitoring mental health status and providing supportive counseling; crisis intervention; arranging transportation for appointments and social outings; documenting services and maintaining medical records; developing staff and client schedules; and attending trainings and team meetings.
