Job Summary
Conduct personal care tasks including bathing, grooming, dressing, toileting, and mobility assistance for clients in their homes. Prepare meals according to dietary needs, perform light housekeeping, and monitor health or behavior changes to report to the healthcare team. Follow care plans, maintain accurate documentation, and communicate with families and team members while adhering to company policies. Requires high school diploma, reliable transportation, and ability to travel to client homes within the service area. Flexible for various shifts including weekends and holidays. New PRN rates start at $18/hour plus travel increase if driving more than an hour.
