PRN Patient Success Pharmacist

Process prescriptions in accordance with organizational policies and applicable laws. Support the Patient Engagement Team with clinical education and patient inquiries, assisting patients with questions about side effects, medication administration, and general usage. Handle prescription transfers, physician calls, data logging, and drug utilization reviews while serving as the primary clinical liaison between Patient Engagement and Pharmacy Operations. Collaborate with healthcare professionals to ensure the highest level of patient care, resolve patient issues, and maintain effective communication across all departments. Update patient records, including allergies, medical conditions, and medication lists. Participate in on-call rotations as assigned and complete continuing education requirements. Ensure compliance with all applicable accreditation standards and meet or exceed department productivity and quality metrics.
