Job Summary
PRN On-Call Licensed Vocational Nurse (LVN) in hospice providing nursing care in patients' homes under RN supervision, coordinating with the Interdisciplinary Team, delivering and documenting care per the plan of care, and participating in hospice QAPI. Requires 2+ years LVN experience in clinical care and 1 year full-time hospice, graduation from a Diploma or Associate School of Nursing, active LVN license, and a valid driver's license. Must be able to travel to patient homes, perform in-home nursing duties, maintain infection control measures, and communicate effectively with patients, families, and care teams.
Required Qualifications
- Two (2) years experience as a Licensed Vocational/Practical Nurse in a clinical care setting and one (1) year full time hospice.
- Graduate of an accredited Diploma or Associate School of Nursing.
- Current State license as a Licensed Vocational/Practical Nurse; Current driver’s license
