Job Summary
Seeking a skilled PRN Nurse Practitioner to deliver high-quality patient care, manage complex clinical cases, perform health assessments, coordinate care, and collaborate with healthcare professionals in a flexible, autonomous role.
Required Qualifications
- Master’s degree in clinical nursing from accredited Nurse practitioner program
- RN license in SC and a Family certification from Nurse Practitioner program
Desired Qualifications
- 3-5 years nursing experience with Nurse Practitioner experience preferred
- 1 year of experience working with a frail or elderly population preferred
- Excellent organizational skills
- Attention to detail
- Verbal and written communication skills
- Knowledge of physical, mental, and social needs of frail older adults
- Effective skills in physical assessment and chronic disease management for frail older adults
- Ability to work within the Interdisciplinary Team (IDT) setting
- Current CPR/BLS certification required
- Must provide a valid driver’s license and auto insurance
- Ability to pass drug screening and background checks
- Up-to-date required immunizations (TDP, MMR, Varicella, FLU, and Pneumonia) and physical exam or statement of good health
- High level of autonomy for clinical and management decisions
