Job Summary
Provide social work services to patients in their residences on an intermittent basis under physician orders and the direction of the Director of Clinical Services. Coordinate agency services with community resources, develop care plans, and assist team members in understanding social and emotional factors related to health problems. Document all patient and family services, prepare clinical progress notes, and participate in patient care conferences and discharge planning. Safeguard patient information, report accidents or unusual occurrences immediately, and maintain current knowledge of Medicare Conditions of Participation. Maintain regular attendance and punctuality while working indoors in agency offices and patient homes, with travel to/from home visits as required.
Required Qualifications
- A master's or doctoral degree from a school of social work accredited by the Council on Social Work Education
- One year of social work experience in a health care setting
- A baccalaureate degree in social work, psychology, sociology or other field related to social work
- At least one year of social work experience in a health care setting
- Two (2) years of appropriate experience
- Acceptance of philosophy and goals of Agency
- Ability to exercise initiative and independent judgment
- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal
- Ability to perform the following tasks if necessary: participate in physical activity and to do bending, lifting and/or standing on a regular basis
- Light lifting may be required
- Ability to work for extended period of time while sitting or standing
Desired Qualifications
- One (1) year experience in home care
