Job Summary
Delivers patient care under RN or clinic supervisor oversight, coordinating and connecting patients with specialists and facilities. Provides evidence-based nursing care at the highest community standard, including phlebotomy and immunization services. Follows up with patients and guides them through the process while modeling Crossover's values within a primary care, patient-centered home model. Requires standing, walking, and sitting for extended periods, with occasional lifting up to 50 lbs. Full BLS certification and three years of clinical experience required.
Required Qualifications
- Licensed Vocational Nurse from an accredited school
- Minimum of 3 years of clinical experience
- Current BLS (Basic Life Support) certification
- Requires standing, walking and sitting for extended amounts of time
- Occasionally lift and carry items weighing up to 50 lbs
- Manual and finger dexterity and hand-eye coordination
- Includes full range of body motion including potential of handling and lifting patients
- Requires corrected vision, hearing and speech within normal ranges
- Must be able to effectively communicate with patients and team members
