PRN LPN Visit Nurse

Provide in-home direct patient care as an LPN Visit Nurse per the prescribed plan of care, performing skilled assessments, developing and implementing comprehensive care plans, monitoring patient progress, and coordinating with other clinicians. Deliver patient-centered care, educate patients and families, ensure accurate clinical documentation in line with agency protocol and Medicare guidelines, and support efforts to decrease readmissions through preventative care and continuity of care. Requires LPN licensure in the practicing state, 1 year of nursing experience, CPR/BLS/First Aid certification, and reliable transportation to home health visit sites.
