Job Summary
Licensed Vocational Nurse (LVN) in Texas for PRN home health. Provide skilled nursing care in patients' homes under RN supervision; assist in identifying patient needs; participate in developing and implementing the Plan of Care (POC); administer treatments per physician orders; monitor patient condition and report changes; maintain accurate clinical documentation; participate in interdisciplinary IDG/IDT case conferences; provide updates to the RN prior to reassessment and discharge visits; uphold OSHA infection-control and safety protocols; ensure patient safety and quality of care; comply with state nursing standards and regulatory requirements. Requires valid Texas LVN license, ability to work independently within a clinical team, reliable transportation, and English communication. Required Qualifications
- Licensed Vocational Nurse (LVN) in Texas
- Minimum of 1 year of home health experience preferred
- Ability to follow and implement a Plan of Care under RN supervision
- Strong clinical assessment, documentation, and communication skills
- Ability to read, write, and communicate effectively in English
- Reliable transportation with valid driver’s license and current auto liability insurance
- Experience working with interdisciplinary teams in a home health setting preferred
