PRN Licensed Vocational Nurse | Home Health (23037)

Provide skilled nursing care to patients in the home setting under the supervision of a Registered Nurse; assist in identifying patient physical, psychosocial, and environmental needs; participate in developing and implementing the Plan of Care (POC) in collaboration with the RN; administer treatments and interventions in accordance with physician orders and agency policies; monitor patient condition and report changes to the supervising RN; maintain accurate and timely clinical documentation in compliance with regulatory standards; participate in interdisciplinary communication and weekly case conferences (IDG/IDT); provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visits; follow and maintain infection control and safety protocols (OSHA standards); ensure patient safety and quality of care at all times; comply with all state regulations and professional standards of nursing practice; participate in required in-service training and maintain clinical competency; meet productivity expectations and ensure documentation is completed within required timeframes.
