Job Summary
Provide skilled nursing care in a client's home according to the plan of care, administering medications and treatments as ordered. Monitor client conditions, report changes promptly, and support individuals with chronic or post-hospital needs. Deliver client and family education regarding safety and condition changes while maintaining accurate documentation in the WellSky app. Communicate proactively with physicians, families, and the care team regarding patient status or scheduling concerns. Work independently within a private home setting while adhering to agency policies and nursing direction.
Required Qualifications
- Active Florida LPN or RN license in good standing
- Reliable transportation, valid driver's license, and auto insurance
- Current hands-on CPR certification, or willingness to complete
- TB test, PPD, or lab work within the last 6 months, or chest X-ray within the last 5 years
- Level 2 AHCA background screening, or willingness to complete
- Ability to work independently in a private home setting
