Job Summary
Assess psychosocial needs, provide counseling on end-of-life issues and grief, and assist families with advance directives, funeral planning, and care transitions. Collaborate with the interdisciplinary team to build comprehensive care plans, connect patients to community resources and benefits, and document all interventions accurately in the EMR. Travel locally to private residences, long-term care facilities, and nursing homes to offer compassionate support across varied environments. Requires a Texas-licensed MSW or BSW with hospice experience, a valid driver's license, and reliable transportation.
Required Qualifications
- Master's degree in Social Work (MSW) from a CSWE accredited program; or Bachelor's degree in Social Work (BSW) or related field (psychology, sociology, etc.) with qualifying experience and MSW supervision as required by regulation.
- Licensed in Texas as an LMSW or LBSW, in good standing with the Texas State Board of Social Worker Examiners.
- Valid Texas driver's license, reliable transportation, and current auto liability insurance required for travel throughout the service area.
- Experience in hospice or palliative care (1+ year preferred) and a strong understanding of end-of-life psychosocial needs.
Desired Qualifications
- Knowledge of local community resources and ability to connect patients and families with support.
- Strong interpersonal and communication skills, including the ability to work with diverse personalities and cultures.
- Comfortable with technology—able to document care and communicate using EMR systems and mobile tools.
