Job Summary

Deliver varied social work services to Hospice patients and their families by conducting initial emotional, spiritual, and psychosocial assessments. Provide ongoing counseling, bereavement services, and community education while participating as a member of the Bereavement Team. Act as an advocate for the patient and family unit, observing and bringing relevant information to IDG conferences. Develop and maintain contact with community agencies to facilitate referrals and document comprehensive assessments clearly. Respond to crisis situations outside of a hospital setting and provide short-term intervention when indicated. Adhere to all Hospice policies and maintain personal and professional development skills.

Required Qualifications

  • MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
  • BSW degree from a school of social work accredited by the CSWE
  • Baccalaureate degree in psychology, sociology or other field related to social work
  • One year of experience in a health care setting
  • Supervision by a MSW from a school of social work accredited by the CSWE with one year of experience in a health care setting
  • Current valid driver's license
  • Auto liability insurance
  • Reliable transportation
  • Evidence of annual TB test
  • Evidence of other state-required tests or examinations
  • Ability to work independently
  • Ability to make accurate, and at times, quick judgments
  • Ability to respond appropriately to crisis outside of a hospital setting
  • Acceptance of and adaptability to different social, racial, cultural and religious modes
  • Ability to cope with mental and emotional stress
  • Demonstration of emotional stability
  • Ability to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials

  • Active patient contact within past three years
