PRN Hospice RN Field Nurse

Job Summary
Provide home-based hospice nursing care with an emphasis on community health in patients’ homes. Responsibilities include completing initial patient and family assessments, performing physical assessments and histories, supervising LPNs and Home Health Aides, developing and updating education and care plans, administering prescribed medications, coordinating discharge planning, acting as Case Manager when assigned, and communicating with physicians to align on patient needs. Emphasis on delivering patient- and family-centered care, ensuring therapeutic, preventive, and rehabilitative actions are incorporated into individualized plans.
Required Qualifications
- Graduate of an accredited school of nursing
- Active RN licensure
- 1-2 years of recent acute care experience in an institutional setting
- Bachelor’s degree with one year home health care experience preferred
- Must be a licensed driver with automobile and insurance in accordance with state requirements
