(PRN) Hospice Registered Nurse, RN

Provide care and services to patients in private homes, skilled nursing facilities, and assisted living facilities as a contributing member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate with social workers, chaplains, volunteers, and aide leadership. Manage patient visits, documentation, and meetings while prioritizing urgent needs through phone, email, and written communication. Maintain independent day-to-day operations with strong time management skills and knowledge of hospice regulations. Requires current Ohio RN licensure, ability to learn electronic medical records, and reliable transportation to drive across the service area. Flexible PRN scheduling available across days, evenings, nights, and weekends.
