PRN Hospice Nurse Practitioner

Job Summary
Conduct routine and emergency face-to-face recertification visits for patients in their third benefit period, including admissions, transfers, and extended stays. Perform bedside palliative care, triage consults by acuity, and provide physical and emotional support to patients and families during crises. Record observations, treatments, and pertinent information while communicating findings with the Medical Director, attending physician, and branch leadership. Prepare accurate patient records, charts, and documents to support medical practice and CMS-compliant billing and coding. Complete 50% travel requirements and adhere to applicable laws and regulations regarding collaborative agreements.
Required Qualifications
- Master's degree in nursing
- Certification through a national Nurse Practitioner Licensing board
- 1+ years of clinical nursing experience
- 1+ year post-graduate clinical nursing experience in the hospice and palliative care field
- Current licensure in the state of practice as a Nurse Practitioner
- Maintain such licensure in accordance with applicable laws and regulations
- Maintain active DEA certification if state allows prescription of controlled substances by APPs
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver's license and insurance
- Reliable transportation to perform job tasks
