Job Summary

Licensed Practical Nurse (LPN) - PRN position providing nursing services in homecare or facility settings under RN supervision. Responsibilities include participating in patient assessment, planning and implementing the Plan of Care, delivering nursing care, documenting care and patient responses, coordinating with the Interdisciplinary Group (IDG), teaching and counseling patients and families, maintaining infection control practices, and ensuring continuity of care. Requires current LPN licensure (compact multistate license) and current CPR; CHPN preferred. Maintains licensure, credentials, and continuing education as required.

Required Qualifications

  • A Diploma from a Board approved nursing program designed to prepare a person for practical nurse licensure
  • Current, active, and unrestricted compact (multistate) license as an LPN
  • Current CPR certification
  • CHPN preferred
  • Minimum 1-year related experience for LDHH and 2 years for other nursing positions; experience in direct care of patients with life-limiting illnesses; hospice and palliative care; home health care; medical surgical
  • Computer proficiency
